Immediate action and lasting improvement

A correction addresses an immediate condition. A corrective-action process also examines cause, assigns actions and responsibilities, and checks the outcome.

Follow your established procedures for affected records and product decisions. Record corrections transparently; never recreate an observation that was not made.

Use a short sequence

  1. DescribeState the finding and the evidence.
  2. UnderstandExamine the cause and the scope.
  3. ActAssign the action, owner, and due date.
  4. ReviewCheck completion and effectiveness.
Check your reasoning
A missing record entry is addressed, but the cause of repeated missing entries has not been examined. What should the team explore next?
Use this as a team discussion

Ask each person to explain the reason for the answer. Then discuss a similar question in your operation and which information or records would help you work through it.

This informal learning activity does not issue a certificate. Arrange project-specific technical questions separately.
